Job Description:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our mission-driven healthcare team.
In this role, you’ll deliver high-quality nursing care, support treatment plans, and collaborate with physicians and other healthcare professionals to achieve the best outcomes for our patients— many of whom are from underserved communities.
Key Responsibilities:
Bring a “ONE TEAM” mindset each day.
Provide direct patient care within your scope of practice, following established protocols.
Patient triage for same-day urgent and post ER/IP admissions.
Patient contact management of post ER and IP admissions
Direct Patient education, i.e. medication, post procedures directives, pre-procedures preparations, changes to clinical procedures or physician orders
Supplemental facilitation of laboratory requests and clinical encounter processing.
Assistance with clinical procedures as directed by licensed practitioners.
Collaborate with the care team to develop and carry out patient-centered care plans.
Daily huddle participation and preparation.
Chart prep oversight and coordination.
Clinical inbox management- telephone encounters, pharmacy requests.
Administer medications, injections, and treatments as prescribed.
Monitor and document patient conditions, reporting changes promptly.
Support providers during assessments and procedures; maintain accurate EMR documentation.
Educate patients and families on medications, care plans, and healthy lifestyle choices.
Maintain a safe, clean, and organized care environment in compliance with infection control standards.
Offer guidance on health maintenance, disease prevention, and case management when needed.
Quality Initiatives Management & documentation control
EDAPS & HEDIS quality Gap management
Coordination with Quality Team Nurses and Clinical Team to manage center initiatives and projects.
Utilization management tasks for the center
Required Qualifications:
Current LPN license.
Minimum 2 years of direct clinical experience (e.g., triage, EKGs, phlebotomy, medication administration).
Experience with Electronic Medical Records (EMR).
Strong teamwork skills and a high level of engagement.
Ability to work effectively in a fast-paced, high-volume environment. Passion for improving patient experiences and outcomes.
Preferred Qualifications:
Bilingual skills.
Knowledge of HEDIS measures.
Familiarity with state-specific primary care requirements.
Understanding of clinical operations.
